Door het invullen van onderstaand formulier kunt u uw abstract indienen voor het VRA-Colloquium op 30 maart 2012 te Arnhem.

U dient uw abstract in het Engels aan te leveren. De presentaties zijn in het Nederlands.

Deadline indienen abstracts: 8 januari 2012

Controle
U ontvangt gelijk een kopie van uw formulier op uw eigen e-mailadres. Indien u geen kopie heeft ontvangen is het indienen niet goed gegaan. Neemt u dan contact op met het VRA bureau via weco@revalidatiegeneeskunde.nl.

Extra controle
Als extra controle of uw formulier ontvangen is door de WeCo krijgt u binnen één week een bevestiging van het VRA bureau. Mocht u deze niet ontvangen dan is het indienen niet goed gegaan. Neem dan ook contact op met het VRA bureau.

Meerdere abstracts
Mocht u meer dan één abstract indienen, dan dient u ook voor alle abstracts een kopie te ontvangen en een bevestiging van ontvangst van het VRA-bureau.

Format
In deze bijlage staat het format dat dient te worden aangehouden om het abstract in te dienen.

Inschrijven
Indien u uw abstract wilt presenteren op het VRA Colloquium vragen wij u zich in te schrijven. Het inschrijfgeld bedraagt 25 euro. Dit bedrag is inclusief: deelname aan het congres, toegang tot de dierentuin, lunch, koffiee/ thee en een parkeerkaart. Na inschrijving ontvangt u in week 12 een entreebewijs en een congresbrochure per post. De inschrijving start half januari.
